COMMITMENTS

The Atlanta Hypertension Initiative (AHI) unites more than 90 organizations across metro Atlanta in a shared effort to advance equitable hypertension control. Organizations across health care, public health, and community sectors, among others, are making measurable commitments to advance hypertension control in metro Atlanta. Their efforts — from launching outreach programs to improving clinical practices — are creating momentum for a heart-healthy Atlanta.

AHI’s Commitments Framework follows a two-part strategy:

 

Empower Individuals and Communities

Commitments in this area focus on empowering individuals and communities with knowledge and resources to manage their blood pressure with confidence. Examples include:

  • Disseminate clear, evidence-based health information
  • Provide health education in trusted community settings
  • Link patients to community and social resources
  • Promote self-blood pressure monitoring
  • Participate in hypertension control training

Equip Health Care Providers

These commitments focus on strengthening clinical systems, data collection, and patient partnerships to drive better blood pressure control. Examples include:

  • Develop a clinical quality improvement (QI) plan
  • Implement blood pressure measurement improvements
  • Strengthen clinical practices for hypertension management
  • Use data to promote improvement and accountability
  • Link patients to community and social resources
  • Implement self-blood pressure monitoring
  • Promote hypertension control training
